In order to participate in athletics, student athletes and their parents/guardians must annually review information on concussions and sign a statement acknowledging receipt and review of the concussion information.
Parents/guardians and student athletes must:
1. Review concussion information by placing your student name and student ID.
2. Sign an acknowledgment letter. After the video, a parent/athlete letter will appear. READ, PRINT and SIGN the letter acknowledging concussion information. This letter must be returned to the school’s athletic director before the student athlete is allowed to participate.
3. Print off a Fact Sheet for your information on concussions.
4. Review additional Web sites on concussion education.
